Intentional walk with bases loaded and being two runs up with two outs in the 9th. Happened twice I think? Both times walked a .300-ish batter, followed by a strikeout against a less prolific batter.

Wasn't really a "play" though. Just an incredibly unlucky bird. Still, I can see why you'd expect something like that to be in here.
